We’re back to share part 2 of our deep dive interview with Brendan Keeler, Interoperability Practice Lead at HTD Health and prolifc health IT writer on his substack healthapiguy.substack.com. In part 2 of our interview series, we do a really deep dive into the nuances of interoperability in healthcare.
But these health IT leaders feel far from empowered and choiceful as consumers in todays EHR vendor monoculture, Harm Scherpbier, MD, explains in his book , Unvendor. It is not yet something you would find in a typical EHR, although some of the largest firms might say its part of their future roadmap. So Im not anti-vendor.

Has momentum shifted to companies more on the digital health front vs. EHRs? He also talks about how HITECH made health IT and EHRs huge 15 years ago, but ponders whether today is more about digital health than EHRs. Epic, of course, has always been private. Hospital IT was big 15 years ago around HITECH, of course.

But the technologies were in no way appropriate to actually deploy in the real world, so things fell into place nicely — success at the Connectathon allowed EHR vendors to claim high ground on interoperability, but without any risk that it would actually be implemented for real. One issue was technical complexity for sure.
On January 10, 2019, Jim Cramer of CNBC’s Mad Money recommended that Apple buy Epic, a market leader in EHRs. A few of my favorite comments were: “Jim Cramer needs a crash course in FHIR standards” from the wonkier section of peanut gallery. I found this tweet of mine from 2009 about Apple and Epic’s collaboration for a mobile EHR.
The Sequoia Project, a spinoff of a prior EHR network, was chosen as the governing body for TEFCA. Today TEFCA only supports document exchange and frankly largely within legacy EHR vendors who have familiarity with the IHE protocols. Of course, the big question is, what is the public benefit?

During the course of the pandemic, 1,400 caregivers have been trained and have performed some manner of virtual visit. Investing in consolidation. Currently, 10% of ambulatory visits are video-based.
Of course, this is the initial list of QHINs. Most EHR vendors I’ve talked to have planned to be part of the network through someone like CommonWell (see MEDITECH’s QHIN and TEFCA approach here ), but I’ll be watching to see if that changes as QHINs develop.
In 2009, the Health Information Technology for Economic and Clinical Health (HITECH) Act was created to promote and expand the adoption of health information technology, specifically, the use of electronic health records (EHRs) by healthcare providers. However, this one about cumbersome EHR really stood out to me. Reminds me of the EHR user conference recently where they said that simplicity is safety in healthcare. One of the challenges with drug data is cumbersome EHR…not going to explain that since you all know. The list of medication challenges in healthcare is long.
