Perhaps the best definition of patient safety is that of the World Health Organization. In advertizing their software, many companies equate hospital readmissions with a patient safety metric though strictly speaking it isn’t by the definition above unless it is related to a complication of treatment rendered during the prior hospitalization.

Electronic health records (EHRs) and personal fitness trackers have helped create awareness through use. According to most recent statistics from the Office of the National Coordinator, use of EHRs has increased from 20% in 2004 to 87% in 2015. EHRs were designed as documentation centers for billing and regulatory purposes.

It had thorough examples and some nice graphical depictions of delivery systems and budding ACOs taking on the challenge of marrying encounter data from claims systems with clinical data from EHR to create “a 360 degree view of the patient.”. But two questions arose as I finished reading. First of all, the article was from February 2013.

The definition provides only a general description, leaving a lot of possibility for interpretation and application down the road. Yet the definition above makes PGHD seem to carry more of an health risk assessment (HRA)-like utility, where patients fill out a survey and have it sent to their doctors in advance.

One use of MHD is as a front API to XDS, but the profile can also be used for point-to-point PUSH (similar to XDR), or access to a Community (like XCA), or just as a simplified API to an EHR or a Direct HISP.
